You May Qualify If You:
- 62+ Years Old
- Reside in 14-county service area
- Meet HUD income guidelines (see below)
Home Modification Examples
- Grab bars and handrails
- Wheelchair ramps
- Improved lighting
- Raised toilet seats
- Tub modifications
- Accessible faucets and lever-style door handles
- Smoke and carbon monoxide detectors
- Other safety and accessibility improvements

- Rental Home
- Mobile Home
Application Checklist of Necessary Information
List of Services

Must Provide Proof of 62 Years of Age or OlderList Item 1
- Such as Driver’s License, US Passport, or any other form of government ID with a birthdate.

Proof of Utility/Electric ServiceList Item 2
- Such as monthly electric bill from Georgia Power, Mitchell EMC, or Local Utility Service Provier

Proof of Household IncomeList Item 3
- Such as verifying income from Social Security, SSI or Retirement Benefits, Tax Returns and Employment Pay Stubs of all persons employed that reside in the home— the last four (4) weeks of payroll check stubs
- A copy of Prior Year’s Tax Return

Proof of Property Ownership or Rental PropertyList Item 4
- A copy of Warranty Deed
- Most Recent Mortgage Statement (must be submitted by landlord if applicant is a renter)
- A copy of Rental Lease Agreement (if applicable)
- Landlord’s Consent Letter (if applicable)

Proof of Property Taxes
- A copy of the paid 2025 Property Tax Receipt (if applicable)

- Signed Including requests for verification and authorization for release of information forms.
